Previous studies have demonstrated that transcranial direct current stimulation (tDCS) can inhibit interhemispheric communication by changing cortical excitability asymmetrically in the stimulated hemisphere. Other studies have suggested that a non-physiologic amount of lidocaine may interfere with tDCS effects by acting as an antagonist to the excitatory effects of anodal tDCS. The purpose of this study was to investigate the effects of lidocaine on anodal tDCS induced cortical inhibition and coherence. In 14 healthy volunteers, the effects of 5% lidocaine were studied using paired pulse transcranial magnetic stimulation (TMS) with interstimulus intervals of 5, 10, and 20 ms, as well as resting motor threshold (RMT) and intracortical inhibition (ICI). The excitatory effects of anodal tDCS were assessed with measures of motor evoked potentials (MEPs) and motor thresholds. Twenty milligrams of lidocaine significantly increased the RMT, MEP amplitude, and ICI of the stimulated hemisphere, whereas it decreased the MEP amplitude of the non-stimulated hemisphere and motor threshold of the non-stimulated hemisphere. However, the application of 5% lidocaine in the M1 did not have any significant effect on the inhibitory effects of anodal tDCS as measured by ICI and MEP. The effects of lidocaine on excitatory effects of anodal tDCS was similar to previous studies. These findings suggest that non-physiologic lidocaine application may interfere with the excitatory effects of anodal tDCS and should be taken into consideration when tDCS is applied in an actual clinical setting.Q:
